  1. Small businesses are usually regulated at the state level. I suspect you need to contact your Secretary of State to do this. Search your state government website. If you employ someone you will need a state and federal employer identification number unless they are contract/self employed.
  2. If you have employees, you need a taxpayer identification number, usually in the form of xx-xxxxxxx. You can get one online in about fifteen seconds from the IRS at IRS.GOV. If you don't have employees, it's your choice. You fill in your Sch-C with your own social security number, as your tax identification number. This is for federal. Your state may, or may not, have different requirements.
  3. You are only required to have an Employer Identification Number (EIN) from IRS if you have employees and/or contractors or want to open a bank account in the business name. Your State may require a number or a DBA. If it is just you working and using your own accounts etc, their is no PIN or number required by the Feds other than your SSN. You need to file Schedule C with your tax return and Schedule SE. There may be other forms required depending on your activities during the year. See an Enrolled Agent (E.A.), CPA, or Accredited Tax Preparer. If you live in California or Oregon, a licensed preparer should be able to handle it. I provide a link to the IRS info on self emplyment: http://www.irs.gov/businesses/small/article/0,,id=115045,00.html
  4. Assuming that your business isn't incorporated, the income and expenses will go on your personal return using a schedule C or C-EZ, a schedule SE and a form 1040. You don't need a special pin for that. You should have been making quarterly estimated payments using form 1040ES for federal. If you have a state or local income tax, you probably need to make estimated payments there also.
  5. You are self employed or independent contractor. You will report your income and expenses on schedule C or C-EZ (Form 1040). This income is subject to SE tax at 15.3%. Read: http://taxipay.blogspot.com/2008/04/tax-filing-by-self-employed-sole.html
