What documentation is required for new hires and personnel files?

I am putting together a new hire packet for my company (which is located in North Carolina) to include all paperwork that needs to be completed by new hires. So far I have the following documents: *Original Application *NC-4 Employee's Witholing Certificate (is this required?) *Federal W-4 Form *I-9 Form *Health and Safety Training Log to be completed upon completion of initial health and safety training *Copy of job description What else is necessary or required for the employee's personnel file? Typo: witholding* I forgot: *Signed Acknowledgement Form for receipt of company handbook will be included as well. Forgot another one: We always get a copy of either a driver license, identification card, or permanent resident card, in addition to a copy of the social security card or birth certificate.

  1. Pay day would be nice, breaks and lunch, sick policy if you have one , vacation if avaible, health benefits if offered. Comapny handbook. Obviously bdawg doesn't know what I-9 is
  2. You better get a SS card, birth certificate and picture ID. You need to document if the employee has a legal right to work in this country. If the employee isn't a natural born citizen you need to get a work visa. There are STIFF fines for not documenting employees work eligibility status.
  3. The NC-4 is required. Most states use different withholding forms for state and federal, instead of just using the W-4 form for both. Also, don't forget to report your new hires to your designated new hire directory. All states are federally mandated to report their new hires. http://www.ncnewhires.com
